We believe our patients can
expect from us: a) a high degree of professional skill and ability; b) that
we will keep abreast of advances in the dental sciences; c) integrity to
perform all services to the best of our knowledge and ability; d) an
explanation of our findings resulting from a thorough diagnosis and the
establishment of a fee for the dental care to be performed.
In return we believe we must expect from our patients: a) complete
cooperation in making and keeping appointments. We reserve the
right to charge for missed appointments; b) a conscientious effort to
follow our instructions on home care and oral hygiene; c) recall visits
at such time as indicated according to your individual conditions; d) a
definite arrangement for the payment of fees. A 1.5% charge will
be added to all balances which are older than 30 days from the time the
service was provided. Collections costs will be added to those
accounts referred for collection; e) if it is necessary to reschedule an
appointment, we require 24 hours notice so that the time reserved for
you may be given to someone else.
